This patient is receiving the cytostatic Methothrexate (MTX). This drug is used in high doses as chemotherapy and in low doses as an immunosuppressant. This patient is prescribed the drug for ANCA vasculitis. Even low doses can cause myeloid dysplasia as seen here particularly in the neutrophilic granulocytes and to a lesser extent the monocytes.
