A primary myelofibrosis with left shift and presence of abnormal myeloid cells with a centrally located mature nucleus. In addition, there was an increased number of erythroblasts. Upon physical examination a palpable spleen was noticed, indicating extramedullary production. Molecularly, the JAK2 mutation was detected.
