Primary myelofibrosis with a prominent presence of micromegakaryocytes in the peripheral blood. At first glance, the cells are reminiscent of lymphocytes. However, a portion show a narrow rim of gray-blue cytoplasm rich in purple granules. Other, morphologically similar cells show only very subtle cytoplasmic protrusions. In addition, neutrophils show aberrant nuclear shapes and hypersegmentation.
